How to create a questionnaire in Elofy?

In this article, you will learn how questionnaires work in Elofy!

In order to perform a survey or a performance evaluation cycle, we first need to create a questionnaire. Here is how to create a questionnaire, step by step!

This article is intended for administrators. The creation of questionnaires is restricted to users with the administrator profile, to learn more about it, click here.

Step 01

In order to start creating a survey, go to Settings > Questionnaires.

Step 02

In order to create a new questionnaire, click on + Create Questionnaire:

NOTE - For pulse surveys, there is a specific screen for creating the questionnaire, as you can see in the image below. In this survey type, you can create "opinion scale" questions from scratch, and you can also use the platform's question bank. To understand more about it, read here.

Step 03

The first step is to define the questionnaire's name and then choose for which type of survey it will be used, since the question options change according to the survey:

  1. Surveys: this questionnaire type can be used for any standard survey, such as climate surveys, engagement surveys, etc., as well as temporal surveys and recurring surveys. Read here about the survey types in Elofy.

  2. Performance evaluations: this questionnaire option is specific for performance evaluations by results, feedback or potential. Performance evaluations are an important tool to know and measure the employee's performance in your company, being one of the main features in Elofy.

  3. Polls (surveys): Polls are quick surveys, usually short and objective. When submitted, they appear on the "My Vision" screen just above the pending items.

  4. You Time: You Time is used for a conversation between an employee and manager after a performance evaluation, as a kind of closure. It is similar to 1-1s, except that the flow is different because it requires an acknowledgement of receipt from the employee.

  5. 1-1s: One-on-one meetings are conversations that can take place either between managers and employees, or between one employee with another employee. It is in this type of questionnaire that we create the templates for the one-on-ones, with their respective talking points.

  6. Feedback: in the feedback feature, you can create your own templates for use by employees, according to the company's culture. It can be used both for sending and requesting feedback.

  7. Dismissal: The Dismissal questionnaire is only for conducting Dismissal Surveys, which will be answered by the person dismissed, the responsible manager, and/or the company's HR.

Step 04

To add a question, click on ‘add question’.

The definition of the question type is one of the most important steps in the process of creating a questionnaire, because once you have chosen the question type you will be able to start producing the questions that will be part of the questionnaire.

Once you click ‘Add Question’, a list with some question-type options will be available. As mentioned before, the options that will appear in the list vary depending on the survey type you select.

There are the following question options:

  1. Survey: Single choice, qualitative, rating, opinion scale, and NPS.

  2. Pulse survey: Opinion scale.

  3. Performance evaluation: Single choice, opinion scale, qualitative and objective.

  4. Polls: Single choice, rating and NPS.

  5. You time: Qualitative.

  6. 1-1s: Talking points

  7. Feedback: Qualitative

  8. Dismissal: Single choice, qualitative, rating and NPS.

A quick guide to each question type:

a) Single Choice: You can define and type the options that will appear in the ruler. In this question type, the user can choose one of the available answers. The numbers in percentages next to each answer choice indicate the order in which they will be displayed within the question.

b) Qualitative: Open-ended questions where the respondent can write their own answer.

c) Rating: This is a kind of "evaluation". In the question configuration, you can choose between Stars (Satisfaction), Hearts (Satisfaction) and Emoticons. The respondent will evaluate the question according to the type of rating chosen:

You can choose different scales, for the star and heart options, as indicated in the images below:

d) NPS questions: They are satisfaction questions to identify the promoters, neutrals, and detractors of the company. It has the following scales: NPS 0-10, 1-10, 0-5, 1-5.

e) Opinion Scale: Scale of agreement. You can customize the text of each quadrant of the ruler, parameterizing with the following options: 1-4, 1-5 and 1-7.

The opinion scale is also used in the pulse survey questionnaire.

NOTE - Once the questionnaire is linked to an ongoing survey, you can't change the "Scale of the ruler points" field. In this way, you can only change the scale of the question after the surveys linked to the Questionnaire have the status "Finished".

f) Objective: Objective questions are specific questions for the Performance Evaluation. They work in the same way as the single choice questions, the difference is that they can be set as a ruler for a performance evaluation when we need a different ruler than the one set in the cycle.

The settings are the same as the single choice question settings.

g) Talking points: Talking points are specific qualitative questions for the 1-1's questionnaires. With the talking points we can establish what the topics of conversation are going to be when we are going to have a 1-1 meeting.

Although they are included in the questionnaire, the category, order, mandatory, justification, and question description fields do not work in the 1:1 template.

Step 06

Once you understand how the questions to be added work, now follow the next steps and the questionnaire will be ready to be used in a survey:

  1. Category: Select a category that includes the question. The category is used to segment the questionnaire, allowing better visualization and organization.

    • You can select one of the existing options or create a new category.

  2. Order: It refers to the presentation of the questions in the survey.

  3. Mandatory: By selecting this option, the question must be answered so that the user can finish the survey.

  4. Justification: For questions that include the "justification" field, in other words, a field to explain why the user selected that option, the field becomes mandatory for users when answering the survey.

  5. Description: Describe the question, if you want.

Don’t forget to save it! Once you have done this, the survey will be available in your list of surveys and you can edit it at any time.

NOTE: It is recommended that you do not change a questionnaire during a performance evaluation or a survey, especially if it has already been answered, since adding more answer choices or deleting a question, for example, can cause some data loss.
