Learn how to create questionnaires for Performance Evaluations used for assessment by results, potential and feedbacks.

The performance evaluation can be done by 5 different and optional categories: employee results analysis, competencies evaluation, evaluation by values, potential and feedbacks.

Some of them are previously configured in the company's settings, others require a questionnaire for the performance evaluation.

Evaluations that require a questionnaire are the evaluation by potential and feedbacks. The evaluation by results can be done by initiatives, OKRs, roles, questionnaires or responsibilities (which are previously configured).

In this article you will learn how to create a questionnaire for a performance evaluation and how to set up these three types of performance evaluations.

Step 01

To begin the process, you must first create a questionnaire.

To access the screen and start creating a questionnaire, go to Settings > Questionnaires.

To create a new questionnaire, click on + Create Questionnaire:

Setting up the Questionnaire:

  1. Questionnaire name: Define the questionnaire name.

  2. Questionnaire type: Select the ‘Performance Evaluation’ option.

  3. Status: Select the questionnaire status.

Questions: Add questions to the questionnaire.

Adding Questions to the Questionnaire

By clicking ‘Add Question’, a list with some question options will be available. As mentioned before, the options that will be shown in the list vary depending on the questionnaire type you have selected.

You can choose four different question types depending on the type of Performance Evaluation questionnaire: ‘Single Choice’, ‘Favorability Scale’, ‘Qualitative’ and ‘Objective’.

a) Single Choice: This option is only available in the questionnaire option for the performance evaluation. This option works just like the multiple-choice option. You can define and type the options that will be displayed in the ruler. In this question type, you can choose one of the available answers. The percentage numbers next to each answer choice represent the order in which they will be displayed within the question.

b) Favorability Scale: Scale of agreement.

You can customize the text of each quadrant of the favorability ruler, parameterizing it on a 5-point scale according to your research. Check it out:

c) Qualitative: It is an open-field question, so that the respondent can write their own answer (it is generally used in the evaluation by feedbacks):

d) Objective: Objective questions are specific Performance Evaluation questions. This question type is used when we need a different ruler than the one set in the cycle, as it can function as a custom ruler.

The settings are exactly the same as the multiple choice or single choice question settings, except that the numbers in percentages next to each answer choice now indicate the score for the point.

Step 04

Once you understand how the questions to be added work, now follow the next steps and the questionnaire will be ready to be used:

  1. Select a category: Select a category that covers the question. The category is used to segment the questionnaire, allowing better visualization and organization.

  2. New category: You can create a new category on the "New Category" button and it will be available for selection.

    *Click on the New Category button>Enter the Category name>Save.

  3. Order: Indicate the sequence that the questions will appear in the questionnaire.

  4. Mandatory: Define whether the question will be mandatory or not.

  5. Justification: Specify whether the question will have the justification box or not.

    Note: if you have chosen to make the question mandatory in the item above, the justification will also be mandatory.

  6. Question: Type the question.

  7. Description of the Question: Describe the question, if you want.

Remember to save it! Once you have done this, the questionnaire will be available in your list of questionnaires and you can edit it at any time.

Note: We advise you not to change a questionnaire during a performance evaluation, especially if they have already been answered, because if you add more answer choices or delete some questions, for example, some data loss may occur.

Step 05

Now that your performance evaluation questionnaire is ready, you can select it in your performance evaluation phase, whether it is by results, potential, or feedbacks.
