The dismissal survey is an important resource for generating relevant data about the corporate culture and also for improving the organization's processes. Additionally, the dismissal interview also helps establish a culture of dialogue, which can help lower the turnover rate, since employees will realize that the company appreciates the opinion of the people who are part of it.
This article has been divided into three sections, click on one of the options below to go straight to the desired one:
Creating the questionnaire
First, you must develop a questionnaire to include the survey.
Step 01
Go to settings > questionnaires.
Step 02
In the questionnaires screen, click on ‘+ create questionnaire’.
Step 03
To start creating the questionnaire, follow the steps below:
Name: choose a name for your questionnaire;
Type: choose the Dismissal Survey type;
Status: Check the ‘yes’ option to activate the Questionnaire. If you want to deactivate it later, just search for the name of your questionnaire, click on edit, and change the status to ‘no’.
Step 04
Also, you must define the type of question you want to use in the questionnaire:
a - Single-Choice: You can specify and type the options that will appear in the ruler. In this question type, you can choose one of the available answers. The numbers in percentages next to each answer option indicate the order in which they will appear within the question.
b - Qualitative: Open field question so that the respondent can write his/her own answer.
c - Rating: This is a kind of ‘evaluation’. In the question settings, you can choose between Stars (Satisfaction), Hearts (Satisfaction) and Emoticons. The respondent will evaluate the question according to the rating type chosen.
You can choose different scales for the Star, Hearts and Emoticons rating types, as indicated in the images:

d - NPS questions: These are satisfaction questions to identify the company's promoters, neutrals and detractors. It has the following scales: NPS 0-10, 1-10, 0-5, 1-5.
Add as many questions as you want to your survey:
Step 05
To finish creating the questionnaire, click on Save.
NOTE - If it is necessary to make any changes to the questionnaire, we recommend that you always create a new one, a new survey, so as not to modify the reports with results.
Creating the survey
After creating the questionnaire, it is time to actually create the dismissal survey.
Step 01
Go to the menu Surveys > Surveys. As shown below:

Click on +New survey:

To start creating the survey, follow the steps below:
Name: set the name of the survey;
Anonymus: ou can define whether the survey will be anonymous or not;
Survey Objective: describe the objective of the survey (optional).
Type: select the ‘Dismissal’ option;
Quiz: link the questionnaire that you created in the previous steps;
Situation: select the status as active.
NOTE - You can only have one dismissal survey active at a time.
Step 04
There are some options related to the dismissal survey:
a) Notifications: You can enable the option to send a reminder email to those who have not responded to the survey. Set the period for sending the reminder to non-respondents.
b) Reminder for users: You can enable the option to send an email to selected people when the former employee answers the dismissal survey. You can select up to 5 persons.
c) Introduction: You can set an introduction that will be displayed to the user before answering the dismissal survey.
d) Message: You can send an email to the survey audience.
Step 05
To finalize the survey settings, click Save.
Sending the survey
Once you have created the questionnaire and the survey, it is time to learn how to send the survey to the employee.
Step 01
In the profile, go to Settings > Users:

Choose the user that the search will be directed. Select the highlighted option as per the example below:

A screen will open where some information must be filled in. See the step-by-step guide and follow the instructions:
Dismissal date: enter the date of the Dismissal;
Type of dismissal: define the type of dismissal (Voluntary or Involuntary);
Termination survey will be answered: select the method by which the survey will be answered:
Direct by Elofy: You can select the dismissed person (the user must be active on the platform) or choose a person responsible for the dismissal interview:
*In this option, a pendency will be created in the ‘My Vision’ tab of the selected person.
By external link: If you need the person to answer the survey, but the user is already inactive on the platform, use this option. A link will be sent directly to the email of the dismissed person, according to the email that will be provided:
After entering the email of the dismissed person and clicking the ‘Send’ button, a screen with the link will be displayed. You can also copy and send the survey link directly to the person:
NOTE - After sending the dismissal survey, if the user still has not responded, an ‘X’ will be displayed next to the sending icon, as shown in the image below:
Thus, if you want to resend the survey to the same user, you need to click on the ‘X’ and redo the sending process.
